LEOSA Application

A year or so ago the AFSFA posted in its email newsletter the access link for online downloading of the required application forms for the LEOSA carry permit. i.e. www.leosaonline.com and 1 or 2 others. On 13 October 2015, I attempted to enter the sites given & received a "Page doesn't exist" response. Would anyone out there be able to provide me a current working link in order that I might begin the process? I'm former Air Police, Deputy Sheriff, Corrections Officer & DoD Security Agent. At this point I'm delightfully retired with enough leasure time at my disposal to complete this feat if I can get into the right website for it.
